The quality of a medical evaluation depends in part on the quality of the information available. Completing the appropriate forms before your visit allows the physician to review your history, identify important patterns, and use consultation time more effectively.

Financial Information
Andelana MD Health & Longevity is a cash-pay medical practice. We do not participate with commercial insurance plans or Medicare, and we do not submit claims for office visits, consultations, procedures, supplements, or other practice services.
IV nutrition, hydration services, and any related supplies or laboratory testing are also provided on a cash-pay basis.
Fees are not published on the website. Patients may request the current fee schedule before scheduling so they can review the cost of the visit or service in advance.
Laboratory testing is generally arranged on a self-pay basis whenever possible to provide more predictable and transparent pricing. When testing is performed through Labcorp or another conventional laboratory, a patient may ask whether the laboratory can bill insurance directly; however, coverage, deductibles, denials, and any remaining balance are determined by the laboratory and the patient’s insurance plan, not by Andelana MD.
Payment is due according to the practice’s financial policy. Patients may request an itemized receipt or superbill for their records, but reimbursement by an insurance plan is not guaranteed. To request the current fee schedule please email andelanamd@gmail.com

Supplements
Supplements may cause adverse effects or interact with medications and medical conditions. Patients should use products only as directed by their treating clinician.
PRP and Aesthetic Procedures
Procedure candidacy and response vary. A consultation is required. PRP, injectables, and aesthetic treatments do not guarantee symptom relief, tissue regeneration, or a specific cosmetic result.
Emergency Care
The practice does not provide emergency medical services through the website, email, voicemail, or patient forms. Call 911 or seek emergency care for urgent or life-threatening symptoms.
