At Andelana MD, every consultation is designed to be thorough, personalized, and focused on your individual health concerns. To help us make the most of your appointment, we ask that all new patients complete the appropriate health questionnaire before scheduling their consultation.
Your completed questionnaire allows the physician to review your medical history, symptoms, and health goals in advance, helping ensure you are scheduled for the most appropriate visit and allowing for a more efficient, meaningful consultation.
If you have more than one primary concern, you are welcome to complete additional questionnaires. We also encourage you to gather any relevant laboratory results, imaging studies, medication and supplement lists, and prior medical records that may assist in your evaluation.

After your request has been submitted, our team will carefully review your information to ensure you are scheduled for the most appropriate consultation. Please allow us time to process your intake materials. Once your review is complete, a member of our team will contact you to discuss the next steps, answer any questions, and assist with scheduling your appointment.
